Complete your health survey
We’ll mail you a short survey in the next couple of weeks to better understand your health history and ensure the transition to your new health plan is easy.
The simple questions on the Health Risk Assessment form cover your recent vaccinations, health conditions, and general health status. Just mail your completed form to us (prepaid envelope included).
Call us at 888-850-8526 if you'd prefer to complete the survey by phone. Your answers do not affect your benefits, costs, or eligibility in any way.

Sign up to earn rewards
The Premera Blue Cross Healthy Rewards program gives you the opportunity to earn rewards for taking care of your health.
You’ll be able to:
- Track and report on activities you’ve completed, like going in for your Annual Wellness visit or getting your flu shot. You’ll be rewarded for each healthy step you take. It’s that simple.
- View automated reminders to schedule your preventive screenings and tests.
Once your membership begins, visit premera.com/healthyrewards to activate your account.
Your Premera ID Card
Use your card when accessing covered medical and plan services and filling prescriptions at the pharmacy.
The name of your plan is located on the front.
Your primary care provider (PCP) and customer service number is located on the back along with other important phone numbers.
